Insurance Program:
If we accept your insurance, Allara does not charge a monthly fee.
Patients are responsible for paying their patient responsibility per their insurance company rates such as copayments, co-insurance, or deductibles.
If you need help figuring out your patient responsibility, here are a few tips:
- Check your insurance card or your benefits package for the co-pay rates for a specialist visit and a Registered Dietitian visit.
- If you have a high-deductible plan, you are responsible for the fully billed cost until the deductible is met.
- If you still need help understanding your responsibility, you may call the Member Services department of your insurance company. Typically the number is found on the back of your insurance card.
- Allara’s NPI number is 1700450558
- Allara’s billing codes are:
- Medical Provider Codes
- New Patient: 99204 - 95
- Established Patient: 99214 - 95
- RD Codes
- New Patient: 97802 - 95
- Established Patient: 97803 - 95
The cost of lab work, imaging, prescriptions, and supplements are not determined by Allara providers and will be billed by the third party, such as the lab facility or pharmacy, that provides that service.
Complete Care (Self-Pay) Program:
If Allara does not accept your insurance, we offer an affordable Complete Care self-pay program which covers the cost of all visits with a Medical Provider and a Registered Dietitian.
- $119 per month, automatically billed to your credit card on file each month.
- Memberships begin on the date of sign-up.
- Patients may cancel their membership at any time without a fee. Cancellations must be completed at least 48 hours before the next billing period, or patient will be charged for the upcoming month.
The cost of lab work, imaging, prescriptions, and supplements are not included.
Nutrition Only Program:
Allara also offers a program that includes only visits with a Registered Dietitian, which costs $75 per month with no long-term commitment.
