I've signed up to pay with my insurance, what happens next?

If your plan requires a referral it’s your responsibility to obtain one and have it emailed or faxed to us. Email [email protected] if you have questions about how to do this. 

Insurance plan types and benefits do vary, co-pays, deductibles and co-insurance may still apply. All out of pocket costs for visits not paid by insurance can be submitted for reimbursement to your FSA/HSA accounts.

When will I be charged for my visit?

Typically within 1-3 weeks after your appointment. Allara sends a bill to your insurance company immediately after your visit. Your company processes your claim, and if there is any patient responsibility due (such as a copay, deductible, or coinsurance) your card on file will be charged for the owed amount. You'll get a receipt via email as soon as the charge goes through. 

How much does a visit cost?

This depends on your individual insurance plan.

Medical Visits: We will bill your insurance as a specialist visit. Typically this cost is printed on the front of your insurance card or in your plan details on your insurance company's website. If you still aren't sure, we recommend contacting the member services number on the back of your card and providing the billing codes for Allara medical visits as follows:

  • Medical Provider Codes
    • New Patient: 99204 - 95
    • Established Patient: 99214 - 95


Nutrition Visits: Most plans include benefits for Registered Dietitian visits. These visits may be subject to a copay or deductible. In cases where your plan does not include this benefit, we will give you the option to pay a reduced cash rate which is comparable to a copay amount. To check your nutrition benefits you may call the member services number on the back of your card and provide the billing codes for Allara nutrition visits as follows:

  • RD Codes
    • New Patient: 97802 - 95
    • Established Patient: 97803 - 95



Depending on your insurance plan, there may be coinsurance or deductible fees associated with your appointments, especially earlier in the year before you may have met your deductible and for those who have a High Deductible Health Plan. You can contact the member services number for your insurance carrier if you have questions about where to find these costs. 

How much will labs cost?

Your cost will depend on the number and type of labs ordered, which will be discussed with your medical provider during your initial visit. Many tests are covered by insurance, however all labs are subject to your insurance plan's lab copay and deductible policy. If you are concerned about the cost of labs, please discuss this with your medical provider during your visit and our Patient Concierge team so we can decide on a plan that is right for you.  


We recommend using your insurance plan's preferred lab partner to keep lab costs as low as possible. The lab you choose will bill your insurance carrier for the cost of your labs at the time of your lab visit. Allara does not bill your insurer or collect payment for labs.

Where can I check my copay and deductible amounts? 

Insurance card: Often your insurance card lists the cost of the copay for a specialist visit (it may be labeled as 'SPC').


Online insurance portal: You can log into your insurance company's portal to view your plan details including copays and deductibles. This is typically the most detailed and up-to-date source of your coverage information. 


Explanation of Benefits (EOB): After receiving services you should get an EOB from your insurance company which breaks down what was charged, how much your insurance paid, and what you owe. 


Contact your insurance company: If you can't find the information through other means, calling the member services number on the back of your insurance card can give you the most accurate and specific answers.
